Officially called the National Commission on Terrorist Attacks Upon the United States, but also known as the Kean Commission, this panel held a hearing on the emergency response to the 2001 attacks and devoted a chapter of their much-read final report to the performance of the FDNY, NYPD and other rescue agencies.

The National Institute for Occupational Safety and Health investigates many firefighter fatalities. While NIOSH reports do not name the deceased or their department, the date and nature of the incidents make it clear which deaths are being described, including the following FDNY fatalities:
